You can use the Breach Reporting Module to log both SRA and ICO breaches.
From the Home Page, click Modules and then Breach Reporting.
Note: Depending on your system settings and account type, you may not be able to see the below screen. Instead, upon selecting Breach Reporting Module as above, it may automatically open a breach reporting record.
Complete the Client/Matter Reference, Breach Identified By and Breach Identified On fields.
Next you will be asked to provide the details for the breach you are reporting.
Note: You are provided with a large text box and are encouraged to provide as much information as possible in order to help those investigating the breach.
Note: You can add further notes and attachments at the bottom of the record if needed.
Select the Department, Office and Practice Area to which the breach relates.
Note: If you have Data Inputter access to the Breach Reporting Module, this is all you will need to record. Click Save and a notification will be automatically sent to the Module Supervisors to advise them to log in and review it.
Users with Supervisor access to the Module, you will now be asked to select a Breach Type. You can select either SRA, ICO or both.
Select a Report Status.
Note: You can find more information about the different Report Status’ by hovering over the icon highlighted below.
Give your reasons as to why you have selected that Breach Status.
Enter the Rule(s)/Regulation(s) Breached, if known.
Finally, indicate if Remedial Action is necessary. If you tick the box to indicate yes, a field will appear to allow you to record the Remedial Action Details.
If no remedial action is selected, your record will now be saved.
If remedial action was selected, you will then be prompted to create the Remedial Action Task.
Provide the details of the remedial action to be completed.
Click Schedule Task.
